If you are coming with a referral from your GP, you will need to bring:

*  A referral letter addressed to The Lighthouse Psychology Practice

* Mental Health Care Plan (if you have a copy)

* Medicare card

* Savings/ Debit card

* Credit card (if paying by credit)

If you are attending an appointment as a private patient, you do not need to bring anything with you, other than cash or card.

If you have any reports/ documents from other treating specialists please bring these along with you and we may ask you if we can retain copies for our records.

Your Psychologist may also ask you to bring an exercise/ note book to sessions.
